What OCTs Are For
An OCT earns its place two ways: it finds what fundus photography cannot see, and it gives you a number you can trend. Retinal nerve fiber layer thickness, ganglion cell analysis, and macular thickness are most useful because you can put today's scan next to the one from a year ago and see whether the patient is actually changing.
That has a direct consequence for buying a refurbished unit. Progression analysis depends on a normative database and on scans being comparable, which means switching platforms resets the clock on your existing patients. If you already run a Cirrus and your patients have years of Guided Progression Analysis behind them, staying inside the family is a good idea.
Spectral-Domain OCT, Scanning Laser Platforms, and OCT Angiography
Spectral-domain OCT. The bulk of this category. Zeiss describes the Cirrus HD-OCT platform as acquiring OCT data roughly 70 times faster than first-generation time-domain technology, at 27,000 A-scans per second against 400 A-scans per second for time-domain.
Scanning laser plus OCT. The Heidelberg Spectralis platform combines scanning laser fundus imaging with simultaneously acquired OCT, and it is modular by design. The base Spectralis OCT can be expanded with BluePeak autofluorescence, the MultiColor Module, the OCT2 Module scan engine, and the Glaucoma Module Premium Edition. The OCT Angiography Module can be added to units built on the OCT2 Module. The Spectralis HRA+OCT adds dye-based fluorescein angiography alongside the OCT, plus the Widefield Imaging and Ultra-Widefield Angiography Modules.
OCT angiography. OCTA images retinal and choroidal vasculature without dye. On this page, OCTA comes from the Zeiss Cirrus 5000 AngioPlex, the Zeiss Cirrus 6000 when it is AngioPlex-equipped, and the Optovue AngioVue. OCTA is a licensed capability, so confirm it is active on the specific unit. Optovue states that AngioAnalytics measures foveal avascular zone parameters and vessel density across the superficial and deep vascular complexes, with multi-scan analysis for visit-to-visit change. OCTA and dye-based fluorescein angiography are used for different things: FA shows dynamic leakage, pooling, and staining, which OCTA cannot.
Combined OCT and fundus camera. The Topcon 3D OCT-1 Maestro pairs a spectral-domain OCT at 50,000 A-scans per second with a full color fundus camera in one body. It is automated down to a single touch for auto-alignment, auto-focus, and auto-shoot. This is the first-generation Maestro, not the current Maestro2, so confirm the generation and the software version on the unit you are quoted. The Optovue iVue takes the modular route: start with the OCT and add the iCam fundus camera later. For a lane that is short on space or short on trained staff, one instrument doing both is often the deciding factor.
Sorting Out the Zeiss Cirrus Model Numbers
The Cirrus naming is the single most common source of confusion in this category, so here is the short version of what is currently on this page.
Cirrus 400. The entry model, built around core OCT function for a smaller budget. It produces the fundus image from the OCT scanner itself rather than adding a separate line scanning ophthalmoscope (LSO). Zeiss notes it shares the same glaucoma and retina analysis package as the 4000 and handles anterior segment imaging.
Cirrus 4000. The premium model of that same generation, with the LSO and the full analysis suite.
Cirrus 500. The next generation of the essential tier, aimed at comprehensive practices. Zeiss added a new OCT camera for faster fundus image refresh, which makes patient alignment quicker, along with Macular Thickness OU Analysis, Ganglion Cell Analysis, and Guided Progression Analysis.
Cirrus 5000. The premium counterpart to the 500. It adds Advanced RPE Analysis, FastTrac retinal tracking for scan-to-scan registration, and Smart HD scan types. Ganglion Cell Analysis is on both the 500 and the 5000.
Cirrus 5000 AngioPlex. A 5000 with OCT angiography.
Cirrus 6000. The current generation, running 100,000 scans per second and imaging up to a 12 mm field in a single scan.
In plain terms: 400 and 4000 are one generation, 500 and 5000 are the next, the lower number in each pair is the essential tier, AngioPlex means OCTA, and 6000 is the newest.
