Refurbished optical biometer price range (typical): $10,000–$40,000

Refurbished optical biometers often range from the high four figures into the five figures, with newer-generation platforms and advanced measurement capabilities trending higher.

Ultrasound A-Scan Biometry Price Range

Used or refurbished A-scan biometry price range (typical): $4000–$10,000+

A-scan ultrasound units often come in at a lower price point than optical biometers, and pricing varies based on condition, probe setup, included components, and calibration status.

What impacts the price most?

Biometry type and system capability. Optical biometers and A-scan ultrasound units serve the same goal, IOL planning, but they are different technologies with different pricing tiers.
Model generation. Newer platforms and upgraded measurement features typically increase cost.
Included software and licenses. IOL calculation packages and upgrades can change pricing. For example, some workflows include Barrett formula suites and toric planning tools as licensed modules.
What’s included. Workstation, printer, accessories, delivery, installation, and training can move the total up or down.
Warranty and service coverage. Longer coverage and stronger service support typically increase the quote.
Condition and refurbishment level. Testing, calibration, recertification, and overall cosmetic condition can impact cost.

What Biometers Are For

Biometry decides the refractive outcome before the surgeon ever touches the eye. Axial length, keratometry, anterior chamber depth, and lens thickness feed the IOL power formula, and a small error in any one of them shows up as a refractive surprise at the one-day post-op.

The formulas your surgeons run, whether that is Barrett, Olsen, Holladay 2, or Haigis, depend on which parameters the unit actually captures. A system that measures lens thickness and white-to-white opens formula options that an older axial-length-and-K unit does not.

PCI, Swept-Source, OLCR, or Ultrasound A-Scan?

The Zeiss IOLMaster 700 is the swept-source unit in the Zeiss line, and it is the step up from PCI for eyes that are hard to measure. Per Zeiss, Total Keratometry combines telecentric measurement of the anterior corneal surface with measurement of the posterior corneal surface, and Zeiss names Barrett TK Universal II, Barrett TK Toric, and Barrett True-K with TK as the formulas that use it. Axial length, corneal curvature, anterior chamber depth, white-to-white, and pupil diameter are all captured without contact.

Contact or immersion ultrasound remains the answer for the eyes that optical biometry cannot get through. Even practices running a swept-source unit often keep an A-scan in the building for mature cataracts, dense vitreous hemorrhage, and corneal opacity. The Nidek AL-Scan takes a hybrid approach. Per Nidek, it captures six values in 10 seconds (axial length, keratometry, anterior chamber depth, central corneal thickness, white-to-white and pupil size) and offers an optional built-in ultrasound biometer in the same housing.

How Biometers Fit the Cataract Workflow

In a typical pre-operative lane, biometry happens after refraction and before the surgeon's consult, and it needs to be repeatable across whoever is running the lane that day. Automated alignment matters more than it sounds. Per Nidek, the AL-Scan's 3-D auto tracking and auto shot align and capture automatically, so the unit adjusts to eye movement rather than relying on the technician to hold the fixation.

The second workflow question is where the numbers go. If your surgeons plan toric lenses, the biometer is the front end of that chain. Zeiss built the Reference Image option on the IOLMaster 500 as the start of a markerless toric workflow, passing keratometry and reference images through to CALLISTO eye for alignment in the surgical microscope. If that is your intended workflow, confirm the downstream pieces before you buy the front end.
